As of June 1, 2021, I will be considered an out-of-network provider with all insurance companies. At that time, payment will be required at the time of service. I will provide you with a superbill which you can submit to your insurance for out-of-network reimbursement. Until June 1 2021, these are the insurance companies I am contracted with.

As a service to my patients, I can bill most insurance companies for my services. It is the patient's responsibility to verify that my care is covered. I can make no guarantees about insurance coverage. Please call your insurance company to confirm that you have benefits for the date and service that you are seen. Anything that is not covered is the patient's responsibility.
